Creating a Form

You can only create a new Form if there is already a Template within the system for you to use. If you do not have one set up, first follow the instructions in the Create a New Form Template guide

From the Menu click on Content Manager

Click on the Create button and choose New Form from the drop-down menu.

Choose the Template you want to use by clicking the radio button next to its name and then clicking on the Create button.

The Form has been created and you will be taken to the administration pages. You can go ahead and assign users in the same way that you would for activities, pathways, etc. by clicking on the Users tab.

Note: Once a form is complete and assigned to a user, the progress cannot be reset like other content activities. A new form needs to be created from the template and allocated accordingly.We recommend testing a form thoroughly prior to allocating it to users.
